I really need help setting up wireless internet on my dell axim x5!

Im really new to all this so ill describe my problem in as much detail as i can.
I recently got a Dell Axim X5 anc a agere wireless LAN CF card.

after much persistance i eventually installed the card, however, when i try to view an internet page it asks me for a username, password and domain. this is trouble as it is as i have no idea what to enter, but also, i was told i needed to configure the internet settings, and when i try to, it asks for a country code, area code, and a phone number, which again, im stumped on.

i am using a netgear wireless router,
